FUMC Content/Photo Release Form
I grant the First United Methodist Church permission to photograph or video record me and
reproduce any content I have provided in any form. All content can be used for any purpose
related to the church, including: advertising, marketing, promotion, and illustration. I
acknowledge that I will not receive any compensation for such use.
If the person signing is under age 18, there must be consent by a parent or guardian as
follows: I am a parent / guardian of the participant who has signed this release and I agree
to the prompt above.
